I have read La Consolacion College Tanauan’s Data Protection Policy and the Terms therein written, and agree to be bound by them. I understand that the Policy and Terms refer to the personal data of my child/ward.
By continuing, I agree with the following:
1. That Republic Act No. 10173, or the Data Privacy Act of 2012, and other relevant Philippine laws apply to the collection and processing of my child’s/ward’s personal data.
2. That by enrolling my child/ward as a pupil/student of La Consolacion College Tanauan, I am allowing it to collect, store, use and process his/her personal data where a legitimate and reasonable educational interest is used in its determination, as enumerated in the Policy and other similar circumstances.
3. That by providing personal information to La Consolacion College Tanauan, I am confirming that the data is true and correct. I understand that La Consolacion College Tanauan reserves the right to revise any decision made on the basis of the information I provide should the information be found to be untrue or incorrect.
4. That any issue that may arise in connection with the processing of my child’s/ward’s personal data will be discussed and settled harmoniously with La Consolacion College Tanauan before resorting to any further action.
